Individual Plans

Individual plans available to anyone and their family members.  Rates for each plan are determined by age and smoker status.  All family members must complete a health history questionnaire (unless certain exceptions are met) when applying; the plan may be approved or denied based on one’s health score. 

Group Plans

Group plans are available only to businesses.  Some plans will cover groups of one to four employees, but groups of five or more will have additional plan options.  Many group plans offer richer benefits than individual plans; however, they can also be more expensive.  The rates are determined by a variety of factors including the average age + home zip codes of your employees.  Optional benefits such as dental and vision can be added to most group plans.
